Problems on amazon transcribe when generate a transcription with Custom language model and Custom vocabulary

When I created a transcription with amazon transcribe and I select a Custom language model and Custom vocabulary the transcription result is bad, but when I use one of the two separately the transcription resoult is rigth, I would like to know any possible cause of this error
