ALB Routing to Unhealthy Targets

Created a Private API GW, provisioned an ALB pointing to the ENIs for the API GW PrivateLink. The targets are in an unhealthy state, but the API request to the ALB pushes through and returns the expected payload from API GW.
Any reason why ALB is routing to unhealthy targets?
