I want to correctly fill out the LOA (Letter of Authorization) form to port my phone numbers to Amazon Connect.
Short description
You've made a request to AWS to port your company's phone numbers to Amazon Connect. As a part of the porting process, you are now asked to fill out an LOA form from the AWS Support team. When you complete filling out the LOA, return the form to the AWS Support team.
Submitting an LOA doesn't guarantee an approval of your request. Your request can be rejected for multiple reasons.
Resolution
The following section provides some examples of common errors and omissions in the submitted LOA form.
Note: The best practices provided below apply to all countries.
Prerequisite
Before you fill out the LOA, review the section How to complete an LOA in How to port your numbers to Amazon Connect.
Incorrect signature format
You typed your name or inserted a photocopied signature instead of providing a handwritten signature.
Best practice: Make sure you sign the form by hand. However, if you choose to use an e-signature, make sure to use a validated or a verified e-signature.
Incorrect submission date
Date of Signature provided in the LOA was more than 15 days from the date when the LOA was submitted
Best practice: If you plan to submit your LOA today, make sure that Date of Signature isn’t older than 15 days.
Incorrect document format
You submitted the form in an incorrect format. For example, you submitted an LOA as a Word document in a .doc format.
Best practice: Convert the document to a non-editable format (.pdf) after completing it. This protects the form from getting modified by another person.
Mismatched company names
The company name on the LOA form doesn't match the company name on the company's phone bill. For example, you've entered ABC Tech Services Pvt LTD as your company name but the phone bill shows the company name as ABC Services LLC.
Best practice: While organizations do sometimes change names, the data provided in the phone bills or the Customer Service Record (CSR), must consistently match the LOA.
Note: You might decide to retain ABC Tech Services Pvt LTD as your company name. Ask your current carrier to change it in the CSR or phone bill for the upcoming month. Submit this information to the AWS Support team.

Provide a phone bill that's within 30 days of the submission date of the LOA. Usually, the winning carrier, (the AWS partner carrier for Amazon Connect), rejects phone bills that are older than 30 days.

Fill the latest LOA form you received from the AWS Support team. Don't fill any previously provided LOAs as the telecom regulations change frequently for each country.

Create separate support cases for separate carriers. In the following example, Carrier A has two numbers and Carrier B has another three numbers:

Create two separate support tickets, one for Carrier A with its numbers and another for Carrier B with its set of numbers.
To port a specific number or a group of numbers from a series of numbers, get prior approval from the losing carrier. For example, to port ten numbers from a series of 100 numbers, inform your losing carrier before you submit a porting request to AWS Support.
