How are DDoS costs handled on regional API Gateway public endpoints? In the event of DDoS would there be a charge for the additional caching?

AWS charges for the services used. Amazon Shield Advanced provides cost offsetting for DDoS based charges which can help mitigate the costs associated in your scenario. Another approach will be ensuring that you've configured your API Gateway caching to accommodate potential attack behavior using Amazon Cloudfront and AWS WAF. There is documentation here which provides some of those best practices.
