Pinpoint TFN registration keeps getting rejected

I'm trying to register our Toll-free number (TFN) so that we can provide SMS two-factor auth. It stopped working last year after US carriers required registration. I've re-submitted my registration application 5 times now, and every time it gets rejected, saying

Opt-in express consent not compliant

I've read and re-read the SMS Best Practices and made changes to both my opt-in workflow description and opt-in workflow image. The current changes are below.

What am I doing wrong?

Users opt-in explicitly by filling out a form and accepting the terms at [organization].dealpoint.me.

Hello!

The issue here is that you are sending the code before asking consent to send messages to the phone number. The process only needs a slight modification on step 2. I would recommend to add a checkbox on the step 2 screen with a legend that reads along the lines of "I consent to receive SMS messages from App X", and only move to the next step after this checkbox is selected. This is the type of consent TFN Registration is looking.
