MTurk template for Image Captioning

Hello,

seeing the examples at https://aws.amazon.com/blogs/machine-learning/high-quality-human-feedback-for-your-generative-ai-applications-from-amazon-sagemaker-ground-truth-plus/ I was wondering if there is any example of the template used to build the "Image Captioning" interface as the one provided in the link. In the MTurk template example, only simple ones are provided.
