My best guess is instance is wither running in a region not supported by Patch Manager or the patch baseline created in patch manager was configured to deploy to instances in regions other than where your instance is deployed.
You can see in the screen shot below the available regions you can choose to configure a patch policy.

Hello,
Patch manager is able to perform patching tasks on instances which have ssm-agent activated in them.
To confirm if an instance is SSM managed, please navigate to Systems Manager -> Fleet Manager In Fleet Manager, you should be able to see all the instances which are SSM managed.
If your instance is not visible in fleet manager, then please follow this link
