I suspect there is an issue in the setup in salesforce In the AC CTI Adapters - ACLightningAdapter screen is the Amazon Connect Instance set to the correct url for your Connect instance?
Also, you seem to be using some old documentation, I'd use the documentation here and maybe start again? T
ook me a couple of attempts to get it working the first time, having never touched salesforce before :)
