Issues with AWS Connect Account Provision

AWS Connect Instance is not connected to Salesforce. I am getting several provisioning errors and I would like someone to review and inform User account details;

Please elaborate the issue over the support case which you will create with Connect team and provide the necessary details like Instance details, the errors which you have observed.
