Can't launch instance - Server.InternalError: Internal error on launch

I've been trying to launch new instances based on a private AMI (ami-0835ad6f4492077da) but they keep failing with that error ("Internal error on launch"). Some recent instances:
i-0f21584bd84e66240
i-03ed9ab7ac04243cf
i-06b85120bdbda12eb

They stay pending and then eventually shut down and terminate.
Any idea what's causing this?

Looks like this was an issue with EC2 that was fixed.

I am also trying to launch instance using the option "Launch More Like This" for the existing running instance. But each time I launch the instance in US West (Oregon) / us-west-2a, I get error after the instance get launched it shows terminated status after few minutes with following error:

State transition reason: Server.InternalError
State transition reason message: Server.InternalError: Internal error on launch

The instance shows the status as: terminated. There is nothing reported on system log for the instance.

Any suggestions why it is happening and how to find out the reason for this failure.
