Problem solved. Luckily, I have an access key(and found it) for the account and was able to use AWS CLI to deactivate the mfa device.
- Installed AWS CLI
https://docs.aws.amazon.com/cli/latest/userguide/install-cliv2.html - Executed the following from the command line to list the mfa device for current account
aws iam list-virtual-mfa-devices - Executed the next command to deactivate the mfa device substituting my user-name and serial-number for this example
aws iam deactivate-mfa-device --user-name Bob --serial-number arn:aws:iam::210987654321:mfa/BobsMFADevice - Logged-in and assigned a new MFA device.
